What is a Social Worker?

A Social Worker is a registered health professional who works with individuals, families and caregivers to strengthen and develop their skills and ability to use their own resources and those of the community to resolve physical, emotional and social problems that they are experiencing in their lives.

What does F.O.C.A.S. Social Workers do?

Our F.O.C.A.S. Social Workers will:

 Complete a comprehensive psycho-social assessment and provide emotional support for clients and their loved ones.
 Provide strategies to help clients and their loved ones adjust to the changes as a result of an accident and/or a natural aging process/unnatural diagnosis.
 Help clients prevent and/or overcome daily challenges such as depression, cognitive issues, behavioral issues and communication of thoughts and feelings.

When do you need a F.O.C.A.S. Social Worker?

 When an individual ages.
 Once an accident happens.
 When an individual is diagnosed with a progressive illness, substantial changes take place that affects not only the individual but also his/her family, friends and his/her loved ones.
 When the individual changes so drastically due to an accident or natural causes, his/her loved ones may not know how to cope with this change.

F.O.C.A.S Social Workers can also help you and your family by:

 Analyzing assessment findings and identifying personal goals to develop a client-centred F.O.C.A.S. care plan.
 Providing counselling services to cope with the impact of illness and life stressors.
 Providing education and support related to psychosocial, emotional and financial needs.
 Advocating for funding sources and accessible services.
 Providing referrals to community agencies and social services.
